Update: I filled out the form you requested. I also sent in this same information on the website when my order was missing. The... website offered me 60 points on the app, but no option to get a refund or get my missing food. After filling in the informationon the app, I had to call the store to speak with someone about the issue. In all, I spent about 20 minutes trying to figure outa resolution. We normally don't have an issue with this location, but yesterday I placed an order. It showed up and I wasmissing an item. I didn't realize it until the delivery man had already left. I tried to report it on the app, and all the appwould do was give me 60 points for next time for inconvenience. I called the store and asked if I could just get a refund on mycard for the missing item. I was told that they could not give refunds, but could only credit my account for next time. I havenever heard of not being able to put a credit back on a card. I ended up just having the missing item delivered again. It showedup over an hour later and we were done eating by then, so I just put it up for the next day. Very inconvenient. Read more

I called them & placed an order for delivery. After I got done placing my order, she took my debit card number & told me my... delivery time. Fine so far. However, I get a call about 5 minutes later informing me that their location doesn't deliver to mylocation. That is a fact that should have been known long before taking my money. They told me they'd refund my card & that itwould take 24hrs. Over a week & I'm still waiting for my refund. My advice is, make sure to ask them if they deliver to yourlocation before you order & then double check before you pay them. Have a great day everybody! Read more
